Last week my GP surgery rang me requesting I going for my diabetic check up, I'm not diabetic I told them. They requested I go for the appointment anyway in case it was something important. Turned out my diabetic check up was a smear test. Strange.

The following day, I went to the chemist to collect my prescription for my heart tablets, we haven't got any. Great. At the end of the week I asked my mental health team for a supporting letter for my pip claim.

The following week I received a letter from my cardiologist requesting the results of the test they requested in November, what test I didn't know about it let alone have it.

Later that same day I went to collect my supporting letter from my mental health team. I was given a letter a year out of date with the wrong doctors surgery, the wrong address for myself, the wrong medication information, a statement claiming I have no physical health issues despite my new heart problem, very irrelevant information regarding my past, a statement claiming I engage with a service I haven't used for several months and a silly comment about me taking my daughter on holiday for a week to Tenerife, all of which is irrelevant and unnecessary for a PIP assessment as well as possibly deemed fraudulent with incorrect doctors surgery and address.

This letter if forwarded to DWP could suggest that I was trying to fraudulently claim pip benefit as 90% of the information is incorrect. I was promised a phone call from my GP to discuss the unknown test requested by my cardiologist and never received a call and also promised a call back from my cpn to discuss the very incorrect supposedly supporting letter which I didn't receive. All of these mistakes and incompetence's in one week. What has happened?
