"Urgent community response"

This team were superb. They enabled my father to stay at home rather than being hospitalised. He would almost certainly have died from being transferred to hospital or very shortly afterwards but because he was able to stay at home and receive daily visits from the team for 10 days or so, he recovered.

This team is essential for preventing unnecessary admissions, particularly in the elderly. It is however extremely difficult to link up with in the first place. The paramedics told us of their existence but then 111 did not know who they were or how to contact them, and nor did the doctor sent out by 111. As a result we had a desperate weekend trying to access the team and it was only the GP on the Monday morning who managed to put things into action

This team is the way forward or in fact back to care in the community.

Thank you so much for taking the time to tell us about the experience you and your father had with the Mid Hampshire UCR team.

We absolutely value all our feedback and the time you made to tell us how you felt. You are absolutely right when you say we are vital to keeping our older population safe at home and able to avoid a hospital admission. I am so pleased to hear that the team worked to make this happen for your Dad. The result of that as a carer is knowing that he avoided an admission into hospital and the disorientation and upheaval that can sometimes cause.

I really appreciate the feedback regarding the services that we aim to work with. Your comments are so helpful to us and I can feed them back to the service involved who should be aware of our service. I'm sorry if this delayed any immediate care for your Dad. I intend to contact the 111 leads in our locality today and I'd happily update you with the response through this thread.

It sounds like your Dad is recovering. We absolutely love the service we provide and I'm immensely proud of the team. Thank you on behalf of us all.

Good Evening

I wanted to follow up on our change action and let you know the progress made today from your feedback.

Today I was able to send your comments to the North Hampshire integrated care board (ICB) partner involved with emergency and urgent care. A number of actions followed that I hope can improve communication regarding our service with 111.

1. We have re shared our service criteria and contact numbers with 111

2. We have provided a poster for sharing

3. We have re shared our service times and the referal process.

It has also led us to think about the route to take for referals that are out of hours for us ie after 8pm and how we might enable this safely for follow up the next day.

I hope this has given some reassurance that your feedback has been acted on. Thank you again for taking time to tell us where things can be improved for our patients and loved ones. Its truley invaluable
