"Treatment and the new computer system"

About: Radiology / CT Service The Ulster Hospital / Emergency Department

Went to the ulster hospitals A&E recently (new bit ) the wee guy on reception was dead on took my details with a smile. I waited for traiage which was fine told to wait for bloods. I was up with rib pain on the right side.

The waiting room was no different to another day in the ulster, busy.  I waited, then next minute I know I was down in zone orange with a Dr. Dr ask why I was up, so I explain my ribs where so so sore. They tried to examine me but, I was that tender around the middle and in at the bottom of me ribs. They did an ultrasound on my arm to get a line in for IV painkillers.

The wee students that were dealing with me were lovely . The problem was scanning the barcodes they were for scanning. Then bloods come back which, so checked X-ray. The girl and fella in X-ray where dead on so back ground and wait for anti blood clot medicine, before home awaits ct scan so all good.

I phone CT to see when it would be, with history of PE it’s stressful. Told I wouldn’t be getting the scan as my X-ray in Oct was clear but no record of having one done the day before. So I’m not getting scan.

I phoned them back the next day to be told, no you are not on our system . What? I have to be I was there at the beginning of the week. Well back to A&E so I phone them to get more anti clot medication, told a Der will call me. Nope never did and getting close to the time I take it.

So I went over to the Ulster. I get there, staff who greeted me was lovely, older receptionist behind her said I told you not to come over here. What they said was a Dr will call me.

Any way back through triage again and the. Dr called me. Then I waited for a few hours, again told them I need need this injection. Well the wee Dr told me I had to wait like everyone else for this and take a sit so I explain. That’s not the case I’m waiting on extra injections.

Well another Dr came over, talked them through the computer about the injections, not realising I was the patient. Said I should have been given the medication by now. So bless them, they went and got it themselves so I had to wait for a nurse to give me it again etc . 

Thurs I phone ct again to see about this scan, really up to high do, no your not on the hospital system. When I went in during the week, the system had all details deleted and wrong numbers in, not even my number and next of kin was a deceased person who wasn’t even my next of kin from a child, seriously old system was better.

I was already told CT to shove the scan as they was causing undue stress etc again with me. I get another call, at this point I was fuming, they told me the request for CT was never put through until I arrived back to the ED middle of the week. What happens if I did have a clot, no harm, I'm sick of this ill treatment, enough is enough.

I get my scan soon and guess what it’s clear, thanks for the undue stress etc once again. Then I had to walk from one end of the hospital to another for the results seriously that wild .

CT were lovely though. And the Dr in minors that give me my results went up and beyond for me bless him 

But the whole system is awful 😞 at least the nursing staff are not standing about talking about a night out now, they are actually doing things they are meant to .

I would firstly like to thank you for taking the time to share your experience and feedback on the service. I am however sorry that you have not had a good experience on this occasion.

As the Deputy Trust Lead for the CT service I am in the position to explain the normal process for a CT referral. The A&E Doctors are required to submit a request to the Radiology Department, on this occasion I believe that this was a CT to look at your chest for a clot. Following this a Radiologist will determine from the information provided if a scan is required. This type of scan if normally performed within 72 hours of referral if it has been approved. If not approved the referral will be redirected by our clerical team back to the referring Doctor in A&E to notify them.

Based on the information you have provided it does not seem that the original request from your first attendance in A&E was submitted or it was and as a result of the new system it did not reach the Radiology Department. The CT department will unfortunately only have become aware of this when you contacted the CT department to enquire about when the scan would be performed. I will discuss your experience with the clerical team, I feel an improvement we could make would be to contact A&E directly on behalf of a patient to determine if there are concerns over a missing referral. This would prevent future patients being bounced back and forwards between A&E and CT.

I am pleased that you were happy with the service provided to you by the CT staff when you had your CT scan. I will gladly share this with the CT team as this goes a long way towards making the staff feel valued in the hard work they put in.

I am sorry that at this time A&E patients who have come from home for their scans are required to travel across the hospital site for their results. Unfortunately the A&E scanners are located beside the new A&E department, however these patients are reviewed in the minor injuries department.

Thank you for reviewing your service when attending the ED recently. Sorry to hear that you did not have a positive experience but it is good to hear you results all came back clear, following bloods, x-ray and CT scan.

We are currently constantly reviewing processes and making improvements with the new IT system and we appreciate the feedback so we can continue to make improvements. I will link in with admin lead and CT to see what further changes we need to make.

Thank you for all the compliments for those staff who were involved in your care. Our staff do work very hard and are under extreme pressure. The recent months have been challenging with moving to a new department and becoming familiar with the new system within a short period of time alongside ever increasing numbers of patients attending the department.
