Follow up after fall and chest infection

Thank you to all team for supporting me. You have thoroughly investigated my medication changes, ECG needed FLt test due to weight loss sweats, constipation and pain. Supported me with prescription 

Very impressed with the whole team care given and full assessment you gave me. I would like to thank the whole team
