"Given conflicting information"

About: Victoria Hospital / Admissions Unit 1 (Emergency Medical)

(as a relative),

A family member passed away - we were not expecting heroics, and accept that this happened. That is not my concern. The problem was that I specifically asked to be called if there was any deterioration in her condition so that we could be with her.

The nurse on duty had advised that she wasnt yet end of life but did scribble the telephone number I asked her to call on a scrap of paper. There were no calls through the night. I called the ward in the morning for an update to be told that my family member had died.

I was told by one member of staff that she wasn't on her own when she died, I was then informed that she had been checked and seemed settled and when a staff member went back to check her again later she had passed away. So what was the true story?  I was supposed to be getting a call back from the staff nurse but I am still waiting.

First of all I wish to offer my condolences on your loss, this will be a very difficult and sad time for you and your family. I am sorry that you were not contacted and therefore unable to be with your loved one when they died. This must have been heartbreaking.

Communication is key and I am sorry that you experienced conflicting stories around the circumstances this must be frustrating and distressing. I note Lisa Stoke the Senior Charge Nurse has reached out to you to contact her so that she can investigate this further.
