"Thank you for being there"

About: South West & Southampton Division / Frailty Support Team - South West

(as a relative),

My mum was discharged from hospital and her dementia was far worse than when she went in.  We had been family care giving but her symptoms were beyond what we could help her with.

I called the gp and they contacted the frailty team. Louise called me very quickly and arranged to visit us the same day. 

She assessed mum and quickly concluded she needed deeper care than we could give.

She took over and arranged everything and mum is now safely in hospital being assessed and cared for.

I can’t thank her and the team enough as I was lost as how to help my mum.

She now needs nursing home care but I didn’t know how to even start the process. Thank you for being there.
