"Let down by service"

About: Mental Health / Community Mental Health Team

(as a service user),

I have been a patient with child mental health then CMHT once an adult. I feel the care received has got progressing worse over the last 10 year. I've found the staff rude, dismissive, and unable to offer support. I feel staff use patients' mental health struggles to defend their behaviour. Notes uploaded to Emis are inaccurate and documented to justify their position.

Details of some issues below:

On several occasions I have phoned CMHT and explained how much I am struggling, I have been advised I will receive a call back however they do no make contact.

I have been on so many psych meds that my immune system has collapsed. It has taken 3 months to get an emergency appointment for a med review. I have detailed that with the way I am feeling I will not be alive to attend the appointment as I cannot manage day to day.

The psychiatrist only works one day a week so no more appointments can be offered sooner. 

I think, my CPN told the police to arrest me for mental health reasons. The police were furious with this response and assured me I would not be arrested.

My psychiatrist told me to take out a loan to go on holiday as if I didn’t feel better and would die anyway then I wouldn’t have to pay it off.

I have received an occasional health letter for a different patient while they received mine, which is a complete confidentiality breach.

I was issued 2 months of medication by mistake to take as per psychiatry, however no such discussion with psychiatry took place.

I am due to revive physicals every 3 months for meds however no such discussion made. 

I am being asked to re-do an autism assessment as they do not have the supporting documents from my diagnosis 2 years ago. 

My psychiatrist told the police to take me back to the police station, keep me in cells and make me attend court the next day as it was a police matter, they could not help, and they could choose whatever proceedings they wish to pursue. The police were going to release me without charge due to the circumstances surrounding the arrest but could not as I was actively suicidal and not fit for release. I spent a lot of money getting this charge dropped and had many trials pushed back due to my psychiatrist refusing to send the supporting letter in time. 

I have been told by duty to phone the police on the voices in my head and banging on the door.

Crisis have told me to eat a banana and pretend to sleep until I eventually do.

I had phoned nhs24 when I could not cope with paranoia people were breaking into the house, could hear people walking around and really scared. By the time they had called back, I had eventually fell asleep however they then broke the door in as I had not responded. I understand why, however if I phone with issues of thinking people are breaking in, to send somebody to break in, seems extremely counterproductive. 

All in all, my CMHT could not haven been more unhelpful if they tried. On many occasions, they have actively caused me distress, supplied me with excess medication to overdose on, refuse to review meds when 7 other professionals have flagged an urgent need. 

I feel there is no understanding of Autism in mental health, thus no support, or reasonable adjustments made to assist individuals in seeking treatment. 

The icing on the cake for me however is when psychiatry told me I'm a nightmare, which I do not dispute, however it’s harder the care and compassion they claim to value in their service.

Thank you for your response,

I appreciate you taking the time to assure me that the CMHT are here to help and to encourage me to reach out. I do however have concerns regarding the accessibility of this support in practice and feel some of the concerns I have raised need addressed within the service in order to offer the appropriate support and uphold the values of the NHS.

I chose to access therapy privately so I do have supports in place however I realise I am lucky to be in a position to make this decision and I am still facing difficulties with the CMHT in regards to medications and routine appointments.

I find part of the biggest challenge living with mental health difficulties is the level of awareness and encouragement to reach out for support only to be met with a brick wall in accessing such support.

For instance, whilst I thank you for the reassurance, I do feel conflicted as on one hand I am encouraged to reach out, yet my experience of reaching out leaves me waiting in crisis for a call back that I then do not receive. Understandably, after this has happened several times, I find it difficult to believe the CMHT are able to help and I would find actions speak louder than words when it comes to such promises.

I would be keen to know if there are any investigations or actions taken to address some of the concerns I have raised as I do believe there are operational problems that need addressed. In particular, staff not calling back, confidentiality breaches, mistakes in prescribing medications, communication between services, loss of documents, awareness of Autism, and the attitudes of some members of staff - both clinical and clerical.

I will continue to utilise the support I have and advocate for myself and others in relation to accessing support from the CMHT however I do feel more action is necessary from yourself in improving the service as a whole to ensure others do not have the same challenges I have had time and time again.
