"Lack of care"

About: Aberdeen Royal Infirmary / General Internal Medicine Adult Social Care / Care Management ARI Hospital Team

(as a relative),

My mum - who is in her 90s - was admitted to ward 105 in poor health, stayed for 2 weeks, staff agreed that some form of care was required at home for her.

ARI care team advised this must be done by staff at 105, I was told this would be done, it wasn't.

Turns out OHS assessed her as ok to go home. Yet over the next 2 weeks in the ward she deteriorated, developed diplopia after the first assessment, no more OHS assessment for care at home and discharged with the same conditions she was admitted with.

I was not advised of the discharge and she was left in the discharge lounge for 2 hours before I arrived to visit. I called ACC care team who were fantastic and had emergency care the next day, well done, they too didn't understand why 105 didn't deal with this.

This  behaviours has fallen well short of the standard expected.

I am so soryr to hear about your Mum's experience. If you feel able to contact us it would allow us to look in to this in more detail to understand what happened and provide you with some feedback.
