"Concerned about the lack of communication"

About: General practices in Greater Glasgow & Clyde Queen Elizabeth University Hospital Glasgow / Renal Dialysis Queen Elizabeth University Hospital Glasgow / Urology (Ward 11c)

(as the patient),

I have recently been an inpatient at QEUH in Glasgow for 3 weeks.  I was in with an acute kidney injury and spent time in critical care.  I was on dialysis, had many blood transfusions and surgeries.  I recovered enough to be discharged with bilateral nephrostomies still in place and a urinary catheter. 

I have 3 bags draining urine that my wife is having to manage at home.  My discharge letter stated I would be brought back in in 2 weeks time to have further surgeries, as the cause of my blocked bladder and kidney issues are still not understood.  It has now been 3 weeks since discharge and I haven't heard from the hospital, and no-one in the community (GP) can give me any information on what will happen to me going forward.

I am still weak and frail and concerned about the lack of communication.  I feel I am being left to sit in my house with bags attached to me that are uncomfortable, and greatly reduce my quality of life. I have tried to speak with my GP for more information, and have been told I can only get a phone appointment for 2 weeks time.  

The disconnect between acute and community is quite unnerving and my family and I are really concerned for my ability to recover from this kidney injury / bladder problem.  I still have no diagnosis and am in a limbo situation at home with stress building day on day.  I feel like I have been left forgotten about amid the waiting list / bed crisis.
