My husband was referred to the frailty Team in Chandler's Ford Southampton, after a stay in Winchester Hospital, with Sepsis

Although out of hospital he was not picking up and was in a downward trajectory.

What a fantastic group of professional people.

Caring, compassionate, knowledgeable and above all working as a team to keep my husband out of a another stay in hospital and to reach a good quality of life.

Not only were they concerned with as a patient but also they were very conscious of how hard it was on the family.
