"Poor mental health support "

I was diagnosed with autism around 2 years ago and was sent the diagnosis and on I could see Clifton court was sent a copy today I was going thru a mental health talk with a doctor

However this doctor advised me he could see nothing about my autism diagnosis and that he would need to contact the team that did that before anything else could be done now I know as I can see on my copy that the surgery did get sent a copy

I called to find out how to do a subject access request for my information as I now don't know what is going on and have been told I need to do this in surgery in person and fill out a form which instantly made my anxiety spike

Most things I could just fill in a form online and send it off with a copy of my id but the surgery wants to cause further stress by me having to make a totally unnecessary journey to fill out a form

Since getting my diagnosis I have wondered where the support was now I know the doctors didn't have the full information despite me telling them of the diagnosis.

To say I'm disappointed is an understatement
