"Failing care for elderly"

My Nan had a fall in June and required a non-weight bearing bed with a full length plaster cast. This is where the care stopped and seems that nobody wants to help her. I have been trying to get her admitted in to a rehabilitation unit, as being in her 90s and not being mobile she is very weak, but it seems everyone I call like to pass the buck and pass the problem on.

I have contacted social services, the ward she was on, her local GP and other organizations but don't get any further. Our biggest worry is that as she is a very independent lady she will try to walk and cause an injury as she isn't strong enough. We have had concerns about the care home she is in and Nan was safeguarded by a visiting nurse as the care plan wasn't being followed. Nan feels that she has been abandoned and forgotten.
