My husband resides in a care home and became very unwell. Nursing and care staff were attentive.
The GP visited and was concerned and made the referral.
The Hospital at Home team united and assessed him and continued to monitor and care for my husband over the next week; support was also available if necessary over the following weekend.
Due to previous experiences and my husbands condition, I did not wish for him to be admitted to hospital emergency department.
The team provided quality of care in a professional and friendly manner.
They provided me with reassurance and support every day.
It made a significant difference to his care and to me.
I know that the service provided good support to nursing staff and communication was shared following each of the three daily visits.
