"Continuity of care"

I first met Lynsey, one of the Community Mental Health OTs, a couple of years ago. Since we met, she has done everything she can to support me in my recovery, to refer me to appropriate and helpful services, and to offer appointments that work around my schedule. Recently, I moved in with my partner in a different area. I was concerned that such a major transition would have a significant and detrimental impact on my mental health, and asked if there was any way a transfer could be arranged to a local OT to ensure continuity of care. Lynsey had never been asked this question but, instead of saying no, she looked into it and managed to arrange the transfer. While I had a short wait to see the local OT, Lynsey continued phone appointments with me to support me through the move.

In my view she went above and beyond the call of duty, and that has been my experience of her since we met. She is absolutely excellent at her job, and works in holistic and individualised ways to ensure the best outcomes for her clients. Thank you so much, Lynsey.
