"Exceptional, patient-centred care"

(as a relative),

I am writing on behalf of my elderly father. Both my husband and I wanted to highlight the excellent patient centred care my father received  at every step of this journey.

The Hospital at Home team came to my parents' house within 2 hours of the GP referral and did a most comprehensive assessment . What was notable was the warm, friendly and respectful approach of the nurse doing this.

Special note must go to ANP Melanie, she was incredible in her assessment of my Dad and in the immediate management of him when he became less well and her prompt and decisive approach. She stayed with my Dad until the ambulance arrived and was professional, calm and just an exemplar of how I would wish my Dad to be treated. Thankyou very much!

The staff in AMU 2 and A22 in Forth Valley were all incredible .  My Dad is incredibly nervous of hospitals and the entire team went above and beyond for him and our family we are so very appreciative.

Our thanks go to every member of the team who treated my Dad with dignity and as a person and have helped  him return home clinically so much better. The diabetic Specialist Nurse was so clear and informative and really helped my Dad get to be where he wants to be home! 

Too often working in the NHS only issues and negatives are highlighted , we want to highlight exceptional, patient-centred care and the amazing team at Forth Valley who have cared for my Dad .
