"Poor service"

Filled in the online form at 8.03am. Filled it in again with admin query at 11am only to be told my form was with duty doctor. Rang at 2pm to be told it was still with doctor. Rang again at 6pm to then receive an appoinyment with practice nurse the following morning. Had appointment with nurse who issued me with a sick note. I then waited 24 hours and no note. Once again I filled in online form and sick note was sent through.
