My elderly mother-in-law was discharged from Wishaw General Hospital following a brief in patient stay. She clearly required home care to effect a safe discharge as her memory had deteriorated significantly over the last few weeks - prior to admission,  she had no 'official ' care. She was discharged to the care of the relatively new Home Assessment Team.

In general the team have provided excellent care allowing a shorter stay in hospital than if no HAT was available, providing the ideal Assessment opportunity for a frail, elderly woman. The occupational therapist and physiotherapist,  Sarah and Carrie in particular were very professional  and thorough in their assessment of needs for ongoing care. They communicated well and timeously with relatives.  

I believe this Team is an excellent way to avoid bed blocking in hospitals, especially with this client group, and  also provides assessment in the best possible location of a client's own home.
