"Thank you for all the care, compassion and commitment"

About: Adult Community Services / Domicilliary Care Service Enhanced Care / Acute Care at Home

(as a relative),

My Aunt was cared for by the Trust Homecare team for approximately the last 10 years. Over this time she grew to love the carers coming into her. Having the daily support of the carers meant that we as a family had reassurance that my aunt was safe to remain in her own home. She loved having the independence and I do believe having this care kept her going.

The carers did go above and beyond and always ensured every detail in her care plan was carried out. I want to thank all the carers who faithfully cared for her and showed her kindness, keeping her in chat and giving joy into her days.

The quality of care was second to none. My aunt was always glowing and her skin creamed and hair styled. She hated losing her independence but the carers continued to encourage her. You were all amazing in your commitment. Thanks so much for that care, you all played your part in keeping her at home.

Unfortunately just after Christmas my aunt took unwell and the hospital at home team became involved. This service was fantastic and exactly what we as a family needed. My aunt loved her home and never wanted to leave it. Having the hospital at home team meant that we were able to care for her at home until she peacefully passed. From the doctor who came to do her initial assessment through to all the professional staff, nurses, OT, physio, Speech and language, we were met with compassion, care and a genuine concern for both my aunt and myself. This coupled with the Double Homecare team coming in meant that I was able to fulfill my aunt's wishes. In the family's time of crisis I was supported and enabled to keep going.

For the hospital at home team, they only knew my aunt a short time but they cared and we knew she was getting the best care. I had many crisis points but knowing the professional advice was only a phonecall away was amazing. In situations I couldn't deal with the team were straight out. This was such a comfort as I didn't want my aunt to suffer in any way.

As for the Homecare team that came in over this period, many of them knew my aunt for years and really showed such compassion. I know it was equally difficult for them seeing her deteriorate  but they remained professional and made sure she was comfortable and still looking fresh with hair styled, they still made her smile even amidst her illness.

My aunt was such a thoughtful lady and I know she would want me to thank you all for the care, compassion and commitment you showed to us. She hated being a burden so thanks from the bottom of my heart for never making her feel that way. We as a family are so grateful to you all. Thank you all for commitment. You are in the careers you were meant to be in. Please accept our thanks and continue to show that care to others.

Hello SG50,

Thanks so much for taking the time to leave a very heartfelt and sincere message. I will be sure to share this with my hospital at home team.

I equally agree that the care teams in the community are invaluable in tending to the day to day needs of our community and whole heartily agree with all your lovely feedback.

Please accept my condolences on the passing of your lovely aunt. I have no doubt that her mannerisms have been passed on to you in your kind words.

Many thanks,

Yvonne
