"Helping an elderly relative"

About: Urgent Community Response / Bassetlaw Urgent Community Response Team

(as a relative),

My aunt was discharged from hospital, she has dementia and not able to live independently. She was sent home with no support plan (she lives alone).

I called the community response team, their help was immediate and wonderful. We could not have coped without their help. They were extremely reassuring, professional and treated my aunt with respect.

What was really thoughtful was the follow up calls when they stepped back to see how things were going and if we were ok. What a wonderful team to have to help us when we are in need.
