About: Community Services (Adult Physical Health) / Community Palliative Service

(as a service user),

Dad received a devastating diagnosis of cancer and in January given a couple of short months to live. Dad decided to remain at home and be cared for by our family. 

The support provided by Chelsea and Tash for both Dad and Mum and us during this difficult time has been outstanding. Often Chelsea would say she is just doing her job, but it was much more. 

We are forever grateful for the support and care for all of us during this sad time. Dad was the heart of our family and his passing has left a huge void in our lives fortunately we have lots of wonderful memories, his sense of humour present till the end. 

We are grateful for Chelsea and Tash helping us facilitate his wish to remain at home so thank you. You’re a credit to the profession and we would have been lost without you.

Thank you so much for taking the time out to tell us about your experiences of the care received from our Community Palliative team at such a difficult time for you all.

We are pleased to know that you had such a positive experience as a family and felt the care and support from both Chelsea and Tash was extended to not only your dad but your mum too. It is heart-warming to know that you felt the team were part of honouring your dad's wish to remain at home but you as a family were a key part of that happening and so you should be proud of facilitating that for your dad. We very much welcome feedback about the team and your feedback will be shared with Chelsea, Tash and all the team.

You sound like you have great memories to share as a family but we also appreciate this is such a difficult time for you. Please do not hesitate to contact if you feel we can help with any further support for you.
