About: South Eastern Health & Social Care Trust / Condition Management Programme

(as the patient),

I attended the Condition Management Programme and found it very informative and the physiotherapist very easy to talk to and listened properly to me. Her advice was brilliant and she was able to tweak it to suit my needs. Very worth while attending. The OT gave me some very good advice as well. I would highly recommend going to.

This programme requires active participation and for tweaks and personalisation to happen the participant has to be practicing and trying out different strategies and activities, Well done to you for all you have achieved through attending the sessions and the changes you have put in place into your daily life.
