"Dads return home from hospital"

About: Urgent Community Response / Urgent Community Response (South)

(as a relative),

Dad has dementia but is able to live at home with carers. He was recently admitted to hospital with Covid and has just been discharged and able to return home.

The process for ensuring he is safe to be back at home has been robust and detailed. His social worker has worked hard to ensure the correct package of care is in place. When he arrived home, Michael (NHS assessor) came to visit to ensure he was happy with the arrangements and that Dad would be as safe as possible. He was very caring towards Dad and completely put my mind at ease that this was the best option for Dad. Putting other people in primary charge of looking after Dad brings many anxious moments for his loved ones but I can now relax knowing that he is as safe as possible and he is happy in his own home. Just saying thank you doesn’t seem enough.
