About: Community Mental Health Teams / Lurgan CMHT Craigavon Area Hospital / Bluestone Unit (Out-patient for mental health) Daisy Hill Hospital / Emergency Department

(as the patient),

I like to thank the Dr Paul Dr king sister Neiva sister Shuna sister Aoife nurses Hannah and Cara patient flow Paula and Dr Garth Hampton who set up my care plan 

I know I was hard with my mental health  to deal way I say sorry for anything I may have said wrong at this time  it was a bad day for my mental health and sizures it I know  I owe a sorry as mental health leave me in bad shape and sizures with the them thanks to all the kind caring staff in ED.

I would also like to thank Tara and Trisen for coming out  and spend time with me Thursday  I am very grateful a manager took time from his dairy to visit and Tara who is my key worker and tower support to thanks 

I also like to thank Kerrie from home treatment for the time taken to call me after my mental health was down I highly recommend  them all for a director thanks
