"Sub-optimal care"

About: Aberdeen Royal Infirmary / Radiology (X-rays and Scans) Emergency Medicine / Minor Injuries Unit

(as a service user),

My husband attended A&E upon referral from GP following attendance at their 'minor injuries' clinic.

He had sustained a 'crush' injury to his thumb resulting in the skin 'bursting' at either side of his thumb- so significant open wounds.

He arrived at A&E at 7pm (left GP at around 6pm & drove directly to ARI), was seen at approx 8pm.

The thumb & wounds were assessed (following removal of a sterile dressing applied by the GP prior to referral)

On removing the dressing, the nurse stuck it on my husband's sleeve, inspected his wound & informed him he would have to return the next day as minor injuries closed at 8pm!

The nurse then removed the old dressing from his sleeve & stuck it back on his open bleeding wounds - I'm really unsure how this would be deemed to comply with infection control procedures?

My husband then had to make an appointment to return the next day - 1st available being 4pm!

Thankfully, for whatever reason, he received a call at 8.30am the next day asking to make his way in.

Thankfully this time, the care provided was far superior & his wound was thoroughly cleaned AND a clean dressing applied after x-ray completed.

If only his initial consultation the day before had been as professional & infection control compliant as this.

Dear tigertx78

I am sorry to hear about the crush injury to your husbands thumb and hope he is recovering well. The Minor injury unit in the Emergency Department, ARI is open 7 days a week from 8am - 8pm. Outwith these times, patients are assessed at triage, and if an injury is not limb or life threatening, an appointment will be given to return. This saves lengthy waits, and although I understand that this may not be ideal, he was seen and assessed in the correct place the following day. I am glad that his wound was thoroughly cleaned and dressed after initial assessment and x-ray in the Minor Injury Unit.

With regards to the dressing being re-applied at triage - I wonder if you could share details with me so that I can investigate further and review the knowledge of wound care and dressing technique. jill.reid3@nhs.scot

Thank you for taking the time to tell us your story.

Kind regards

Jill
