"Help with filling in forms"

One of the Lanarkshire disability forum team Karen visited me at home to help fill out adult disability forms and disabled badge forms, on two occasions. It was great that she was able to come to my home as I find it difficult having to go to places.

Karen was so patient and took her time to explain everything to me and made me feel at ease. She also contacted occupational therapy and done a referral aswell. Karen also contacted me few weeks after to see if I needed anymore help. I was so grateful to get this help as I would never have managed to do it myself.
