I was lucky that a locum was covering for my normal doctor one day. The locum spotted that I hadn't been told to change the way I take my medicines when my symptoms changed. This meant the medicine was not protecting me at all for 12 hours a day. Such a simple thing, but it had left me having seizures that were entirely avoidable. I have now switched hospitals and it is a revelation.
