(as the patient),

Being discharged in pain with no satisfactory explanation. Have a hysterectomy booked in 9 days time on pain medication, and a bladder in retention. Supposed to cope at home with children etc to care for. There is no joined up communication between different shifts or departments and feel my issues have not been addressed or resolved. Very upset and unhappy with level of care.
