"Good team-more communication needed"

About: South West London And St George's Mental Health NHS Trust

(as a student nurse),

The team that I am working with are very good. However, communication between them and their affiliate wards should be improved as this provides a good continuity of patient care and achieve patient recovery goals. At the same time this team should involve patients in planning their care collaboratively.

My name is Jane and I am one of the Patient Experience Leads for the Trust. Many thanks for taking the time to share your thoughts about your placement within the Trust. It is always valuable to hear the experiences of people such as yourselves as it enables us to learn and make changes where necessary. I will ensure that your concerns about communication between teams are passed onto the appropriate staff to enable them to consider ways in which this can be improved.
