My elderly mother had been unwell for several weeks and was referred to specialist Geriatric Team at St John's. Throughout the two weeks Mum was managed by this team her symptoms were investigated and treatment options fully discussed with myself and Mum, whilst the Consultant attended mum and nurses followed up daily with visits. I was aware that there were many more members involved in Mums care planning, pharmacist, Admin and Clerical. They quickly made excellent therapeutic relationships with Mum and I and treated both of us with dignity and respect. Mum has now been discharged back to the care of her GP. I am in no doubt that her return to her normal activity could not have been achieved by admission to an Acute hospital in such a short period of time. Hospital at Home is the Gold Standard for a large number of older people who can be managed at home. Thank you so much.
