"PALS Complaint in progress"

About: Royal Stoke University Hospital

Arrived at A&E approx 11am.

Referred to AEC approx 12.30. Blood tests taken

Seen in AEC approx 16.00

Sent for CT scan as d dimer reading was off the scale.

CT scan completed returned to AEC

18.50 wife complained that I had no pain relief and was sitting in a chair with confirmed DVT left leg

20.30-20.45 called in to see Doctor who told us the original doctor I had seen had collapsed a minute after I had been seen and that I was being sent for blood tests and a CT scan. My wife pointed out I had alresdy these which was obvious by the green cannular in my left atm.

The Doctor left and came back with a condiltant wjo said I was stopping in. I asked if there was a bed and was told no. I asked when a bed would be availible and was told don't know. In that case I'm going homeas I am in severe pain having had no pain relief and I'm sitting in a chair with DVT. The consultant argued and I said if he could gaurantee me a bed there and then I would stah if not then I was going home to bed. He seemed more interested in me signing a form saying I had signed myself out against orders which became more obvious a week later.

Had a phone call from heart and lung Saturday 6th January urgent echo cardiogram required. Booked it for 12 on the 8th January to fit in with my AEC appointment.

Attended AEC 9am Monday 8th blood tests taken. Called in to see doctor when tests came back. Doctor asked me why I was there?. I explained and he said there are no notes on your file on the computer. I have these results but nothing to compare them with. They seem better so off you go. I asked about follow up with the severity and was told he knew nothing and maybe someone would be in touch.

Attended heart and lung at 11.35 a full 25 minutes before my test and booked in at reception. 12.55 my wife realising I am in pain and struggling to breathe goes to reception and ask how long I will be waiting. Only to be told the previous receptionist had not booked me in. When we arrived at 11.35 we were the only ones there at 13.00 the waiting room was full and the receptionist said don't worry we will get him seen sometime this afternoon.

Wednesday 10th attended ct scan on lungs and abdomen, asked who the notes were being sent to and was told nothing on system so they will go to your GP.

Friday 19th recieve a phone call from Silverdale surgery saying it was urgent I attend for tests. Make me an appointment then please " sorry we have no spaces for 2 weeks" after I pointed out she had phoned me saying it was urgent she booked me in to see the practise nurse.

She asked why I was there and my wife reeled off the fiasco and the nurse went paler and paler, then she took my stats and went ghost like. At this point she showed us the 3 lines on the computer that the hospital had put which consisted of. Attended a&e referred to aec. Signed himself out against consultants advice. The practise nurse who in the whole fiasco has been the only truly caring professional
