"Outpatient appointment & COVID-19 questions"

My appointment letter requested that on the day before appointment I should visit the website www.cctayside.co.uk to complete the COVID questions.
I have accessed the site but have been unable to get it to work. I feel more user-friendly access is needed.

I am sorry the website wasn't working the day you were needing to use it. We have checked today that is working and it appears to be functioning well. If this is still not your experience then please don't hesitate to let us know that by responding to this post.
