"Failure to arrange an important procedure"

An outpatient procedure was supposed to be scheduled, having being discharged 3 months ago, but it still hasn't been booked, despite many emails to PALS & chasing the Consultant whose care I was under during the inpatient admission. 

PALS last responded at the beginning of August, saying they'd forward my email to the relevant department to look into, and that was the last I ever heard from anyone & I still don't have a procedure date, or an explanation.
In desperation, and given the severity of my symptoms & very complex medical history, I asked another Consultant I'm under, at a different Hospital, and despite it not being their responsibility, they managed to request & book the procedure in just a couple of days, and it only involved 1 email & 1 phone call to do so. 
This failure is particularly concerning, in view of it resulting in avoidable suffering, completely avoidable harm, and avoidable delay.
