We are very appreciative for the care which our relative recently received from your Acute Care at Home Team.

The various members of the Team who visited and attended her, together with the pharmacists who rang us — were, without exception, excellent. It was a great relief to her to be offered this service, rather than having to be admitted to hospital.

Please convey our renewed thanks to the Team, and best wishes to you all for the successful continuation of this model programme.
