I have significant concerns about this ward A11. Basic care is not being completed such as personal care (no shower for 4 days). Only offered one jug of water during the day is not ensuring adequate hydration. My relative was meant to be having observations 4-6 hourly- this was not being completed - observations at 6.30am only recorded on their chart despite having had a raised temperature and requirement for oxygen overnight.
Elderly people are not being encouraged to mobilise which will ultimately will result in delayed discharges. A stool sample was requested and produced. The specimen was not collected until staff were prompted twice after a further bowel movement. Medication not being administered until 11pm without instructions given on how to take it for example - one tablet is to be dissolved under the tongue and an antibiotic to be drunk with a full glass of water but not meant to drink water for 3 hours before the medication to be placed under the tongue!
Finally, I observed a significant safety concern- medication (including antidepressants) in the drawer next to the bed was not locked. This is a significant risk which should be addressed particularly in a single room where there is evidence that observations have not taken place.
