"Condition missed in A & E"

About: Northern General Hospital / Accident and emergency

(as a service user),

I went to A&E 2 times recently 

1st. Palpitations 

2nd. Severe headache, couldn't lift head, dizzy, broken speech 

I was discharged both times and they said I was fine and it was "just anxiety" and migraine. My ECG did show sinus tachycardia. Why didn't they check my thyroid bloods? 

A week later, a GP rings saying I have an overactive thyroid. 

Why did the hospital miss this?
